Since our founding in 2020, we’ve been dedicated to providing personalized, member-focused care that promotes independence, integration, and dignity for every individual we serve.
Each member in Carroll has a voice in their care plan, empowering them to set personal goals and work towards them with the support of our dedicated team. We work closely with state agencies and community organizations to provide a variety of support services, including:

Our Host Home program in Carroll offers a unique alternative to traditional residential care. Carefully selected and trained local families welcome members into their homes, creating a nurturing and inclusive environment where they can thrive. As part of the Host Home family, members form meaningful relationships, develop social skills, and enjoy the stability of a supportive household.
This approach not only provides personalized care but also fosters connections that enhance the well-being of everyone involved.
Iowa has seven HCBS (Home and Community Based Services) Programs, and we handle one—Intellectual Disabilities (ID). Individuals receiving the waiver are called members, with a tier rate assigned based on their support needs. Those with fewer needs are rated lower, while those needing more support are rated higher. We mainly work with members at the higher end of the scale, many of whom were previously stuck in emergency rooms, jails, or nursing homes, creating significant costs and poor environments. While group homes with 24/7 staffing work, we now focus on the Host Home model, where local families provide full-time care in their homes.
Every member has a case worker who sits down with us to come up with a list of goals and talk about progress. Goals are highly personalized. We have some members whose goal it is to take a shower every day and others who are trying to learn to shop. Every day we get documentation as to whether they attempted their goal, completed their goal, if they required prompting, had any challenges, etc. We also document their community integration but that is not always a written goal. Our host homes are paid and qualify for a federal tax exemption so they are able to make member care their full time job.
Ultimately, our goal is to provide the best possible life for our members and the keys to that are very simple: community integration and dignity. There are countless stories of members with terrible behaviors in their past that have shown complete turnarounds after coming to our agency and we strongly believe that is because we treat them with the respect that they deserve and offer them opportunities to find fulfillment in life. As an agency, one thing we do particularly well is bringing on high quality employees who care deeply about the work that we do. We are incredibly passionate about this community.
