Home Host Provider

As a Host Home Provider, you will create a nurturing, supportive, and home-like environment for adults who need assistance with daily living activities. This role involves integrating an Ability United Member into your family and community, providing a personalized alternative to traditional residential care settings. Your contributions will directly enhance the Member’s quality of life, foster their independence, and encourage active participation in community activities.

Key Responsibilities:

  • Provide a safe, welcoming, and inclusive living environment for the Member.
  • Assist with daily living activities, including personal care, medication management, and mobility support.
  • Coordinate and provide transportation to appointments, social activities, and other outings.
  • Plan and prepare meals that align with the Member’s dietary needs and preferences.
  • Encourage and facilitate the Member’s involvement in community activities.
  • Collaborate with care teams to implement personalized care plans that address the physical, emotional, and social needs of the Member.
  • Maintain regular communication with the agency, providing updates on the Member’s progress and any concerns or needs.
  • Document the Member’s health, activities, and overall well-being daily, ensuring accurate and up-to-date records.
